Primary Tumors of the Small Intestine





Abstract
Primary tumors of the small bowel are uncommon tumors that may be encountered by the surgeon. Their infrequency and the lack of awareness of their existence can result in a late diagnosis and poor prognosis.
Authors experienced 56 cases of primary small bowel tumor during 12 years period in Yonsei Medical Center and the results were as follows;
1) In total 56 cases, 35 cases were male and 21 cases were female and benign tumor was 18 cases and malignant tumor was 38 cases.
2) The mean age was 41 years old; benign tumor 43 years old and malignant tumor 40 years old.
3) The ileum was most common site(42.8%) of small bowel tumor which was followed by duodenum, jejunum and 8 cases(44.4%) of leiomyoma were most common benign tumor among 18 cases and 12 cases(31.3%) of adenocarcinoma were most common malignant tumor which were followed by 11 cases of malignant lymphoma and leiomyosarcoma, respectively.
4) The most common symptom was ahdcminal pain(64.3%) which was followed by nausea and vomiting(25.0%). And the most common physical sign was abdominal mass(50.0%) which was palpated in 44.4% of benign tumor and 52.6% of malignant tumor.
5) The accuracy of preoperative diagnosis was 30% and may elevated to 58% if including suspicious cases.
6) The laparotomy was performed in 32 cases among 38 cases of malignant tumors and curative resection was performed in 18 cases(56.2%) and biopsy and bypass procedure was done in 5 cases(l5.6%). And there was 12 cases of regional lymph node or hepatic metastasis.
7) The postoperative complication was occurred in 7 cases and the wound infection was most common. The operative mortality was 3.1%.
